Fertility Assessment / Psycho-education (includes written report)

Psychoeducation: intended parent(s) using gestational carrier: $250

Psychoeducation: intended parent(s) using donor (sperm/egg/embryo): $250

Insurance

I am unable to bill insurance for fertility assessments/psycho-education.

If you would like to use your insurance benefit to cover our therapy sessions together it will be important for you to double check with your plan about your in and out of network coverage. I am in network with Premera, Regence, Kaiser PPO and OPTIONS, First Choice Health Network (FCHN), Cigna and Uniform Medical Plan (UMP).

Payment

I accept payment with check, credit card (including HSA and FSA cards) and PayPal.

Cancellation Fee

I require 48-hour notice for cancellation with a fee of $150 if we are unable to reschedule within the week.
